Trek Registration Forms

Here is the Registration Packet handed out at the Pre-Trek Fireside on January 8, 2012. 
To Download this file:

4. It is set up to print double-sided. 

All youth and leaders must fill out this packet in order to go on the trek. 

• Page 1: Please fill out immediately and return to your Youth Leaders.  They will turn it into the stake immediately.
• Page 2: Registration Form and Medical History.  Please fill out and return to your Youth Leaders by March 11, 2012.
• Page 3: Photo Release.  Please fill out and return with your Registration Form by March 11, 2012
• Page 4: Pre-Trek Hike info, Foot Preparations, Supply List for you to keep for your reference.
